About:

The Four Villages Community Health Centre provides health care services and health promotion programs through an inter-professional, team-based primary health care model.
Every effort is made to register eligible clients to our practice. When our registration is full and we are unable to accept new clients, your name will be added to a wait list and we will provide you with information for various other resources in the community if necessary until space becomes available.
