Supportive Housing

Supportive housing provides people living with additional medical, emotional or social needs to achieve their recovery and live an independent life with support as they need it.  Providing permanent, affordable rental housing with support services on-site will help people to move out of the shelter system.

Applications for Supportive Housing in Toronto must go through Access Point, which is a centralized application portal for 51 supportive housing providers.

Before applying to Access Point, you will need to know what type of supportive housing program you want. Applicants may be eligible to apply for more than one of the housing support options.

  • Mental Health Supportive Housing Program
  • Problematic Substance Use Housing Program
  • Mental Health and Justice Housing Program

16 years of age or older

Have complex mental health and/or addiction needs.

* Proof is not needed for the application, but you will need to provide medical history and/or diagnoses for program acceptance.

Qualify for a housing subsidy.

Be willing to meet with support staff on a regular basis

Fill out the online application form.

Once your application has been reviewed, Access Point will contact you by phone to discuss your eligibility for services.

There are three Access Point Supportive Housing programs:

  • Mental Health Supportive Housing Program supports people with mental health concerns including addictions with various levels of support
  • Problematic Substance Use Program supports people with severe and active substance use, homelessness, and be a high service user of other community resources
  • Mental Health and Justice Program supports people at-risk-of or experiencing homelessness, have involvement with Criminal Justice system, and referred by organizations working in the justice system

Access Point also provides individual support services for people living in the community with serious mental health and/or addictions.

  • Intensive Case Management supports people with serious mental illness and/or addictions to make meaningful changes. Early Intervention in Psychosis Program falls under this category.
  • Assertive Community Treatment Teams (ACTT) provides treatment, rehabilitation and support to people with a severe mental health diagnosis, a history of hospitalizations and specific need.
